We look forward to meeting you at your first appointment. Please arrive 15 minutes prior to your appointment time so that we can register your information. Your therapist will start your evaluation immediately following this.

Please remember to bring with you:

• Your doctor's prescription
• Your insurance card
• A photo I.D.
• A change of comfortable clothing
• Completed paperwork
